Breast Revision: This athletic lady in her 40s had a prior breast augmentation over 16 years ago. She complained that her breast implants had changed in position over time. She had also developed a hard capsular contracture on the right. The subsequent breast ptosis (or droop) bothered her. Despite daily grueling workouts and peak physical condition, she could not correct her skin laxity issues. Preop, on her right breast, you can visualize well what is commonly referred to as a double bubble or “snoopy dog” deformity. With a double bubble, the breast implant stays high under the pectoralis major muscle and the breast gland and nipple droop off of it creating two mounds. The upper mound was the implant under the muscle and the lower mound was the ptotic (or droopy) breast tissue below it. This can only be adequately corrected with a mastopexy, or breast lift. Here she is less than 2 months after a breast lift (mastopexy) with breast implant exchange (downsize from 450/400cc saline to 350 silicone cohesive gel). She elected to have her c-section scar revised with a mini abdominoplasty and liposuction at the same time. (A full abdominoplasty was not performed because she did not want her tattoo to be compromised).
